Living Well is an 8-week – plus 4 “BONUS
CLASSES” –
nationally recognized health &
wellness training specifically
designed for adults of all ages with
mobility disabilities.**
**Anyone with a disability may sign up,
but materials are geared to people with mobility disabilities.
Classes start MAY 12th 2005 in Cedar

In order to participate fully in the
LIVING WELL Trainings, what special accommodations will
you need (like transportation to &
from the training site, interpreters, materials in accessible
formats, special menu, help writing)?
HOW MUCH DOES IT COST? The Living Well
classes are FREE!! You will get workbooks (a
$25.00 value) & a light supper. We can
provide personal assistance and transportation.
WHEN & WHERE: Classes meet every
Thursday from 6:00 PM to 8:00 PM for eight weeks starting
May 12th 2005. Four BONUS classes will be
held monthly. All classes will be at St Luke’s
Hospital – 855 A Ave NE in Cedar Rapids.
RESEARCH PARTICIPATION: We are doing
research to find out if this training helps improve your
health & quality of life. You will be
invited to take part. You will be asked to fill out a health
survey four times. You will be offered
$10.00 each time you complete a survey form. You may
still take the training free of charge
even if you want to skip the research portion of this project.
CURRICULUM, FACILITATORS & SPONSORS:
The Living Well With A Disability curriculum was
developed by the Rural Institute on
Disabilities at the University of Montana, and supplemented
by the Center for Disabilities &
Development at the University of Iowa. The primary facilitators
for this training session will be Cherie Clark,
Cedar Rapids Coordinator for the Conner Center,
and long-time disability rights activist
in Iowa, and Michael Chalupa, a member of the DD
Council and a P.A.D.S. Executive Committee
Member. Both are certified facilitators for Living
Well. This training is a part of a
research project funded by the Center for Disease Control
(CDC) in cooperation with the Center for
Disability & Development at the University of Iowa, the
Prevention of Disabilities Policy Council,
the Iowa Department of Public Health, and St. Luke’s
Hospital Rehabilitation Services. It is
being conducted & managed by the Conner Center for
Independent Living in Iowa City.
